Failure Reporting And Corrective Action Systems (FRACAS) are an integral aspect of both development and manufacturing operations. A sound FRACAS will ensure that all failures identified during a process are recorded accurately, assigned to an owner, driven to root cause, and corrected to ensure they do not reoccur.

Failure Recording

When a failure occurs there is certain information that must be recorded to ensure the engineer assigned to investigating and correcting the failure mode has accurate information to base their experiments on. Proper training on a FRACAS system is essential to ensuring that all users understand their responsibilities.

Failure Analysis

The failure mode is analysed to determine the cause of failure and associated stimuli to repeat the failure mode reliably. Once the failure mode is understood a team consisting of engineering staff and management must decide on an appropriate severity rating and assign an engineer to implement corrective action. After a period of time, or with training, companies can implement severity ratings for common faults, and the process of failure analysis will shorten.

Corrective Action and Verification

Once an engineer has determined a corrective action to prevent the failure mode from occurring it should be reviewed by the FRACAS team, and if found to be suitable, should be implemented. Once implemented the corrective action must be verified to ensure the original failure mode no longer occurs. All details of the corrective action must be documented, along with the time to repair, the unit details, and the failure details.

Closing the Loop

All failure modes must be documented, along with the associated corrective action, and verification testing. Once documented the failure must be fed back into the design, or manufacturing environment to ensure the mistake is not made again. By closing the loop and preventing the same type of failure mode to occur in future designs we are improving reliability through each product evolution.
